Type your email address instead of copying and pasting it. Pasted text can prevent the form from accepting the entry.
Facebook link not working
Ensure your link starts with the full Facebook website address, such as https://www.facebook.com/facebookname.
Phone number not working
Enter digits only, without any special characters or spaces. For example, please enter your phone number as 1234567890, not (123) 456-7890.

Upload your file as a PDF under 20MB. This format works best with the software.
